Initial encounter is not simply the first visit
In the chapter 19 framework, A generally describes an encounter involving active treatment. D describes routine care during healing or recovery after active treatment. S identifies a sequela, with additional instructions for the residual condition and sequencing. A new provider or a second appointment does not, by itself, determine the character.
Do not apply A, D and S as a universal template. Fracture categories have additional values, and other categories may use seventh characters differently. Read the values provided for the specific code family.
