A category may need more detail
The Tabular List is a hierarchy of categories, subcategories and codes. Some categories are complete codes; others branch into more detailed entries. A character count alone is not enough to decide whether an entry is complete, particularly where a seventh character is required.
Open M54.5 and compare it with M54.50. The first groups more detailed low-back-pain entries. Follow the hierarchy and the official instructions instead of copying a category because it matches the broad search phrase.

A “billable” badge is a reference label
In Code Hub, the billable flag identifies an entry listed as a reportable code in the source code-order data. It does not check a patient record, select the principal diagnosis, or evaluate a payer’s coverage requirements.
Treat the badge as one structural check within the lookup. A code can exist in the classification without being appropriate for the documented encounter. This website does not adjudicate claims or promise reimbursement; its role is to make the published reference easier to inspect.
Unspecified is not automatically incorrect
An unspecified entry is used when the documentation lacks the detail needed for a more specific assignment. An “other specified” entry serves a different purpose: the record contains detail for which the classification has no dedicated specific code. Review the relevant index entry because some categories handle these distinctions differently.
The official guidelines recognize a role for symptom and unspecified codes. Choose the detail supported by the encounter information and applicable guidance. Do not invent laterality, a cause or another clinical qualifier to obtain a more detailed-looking code.
