Excludes1 vs Excludes2: how to read the difference

An Excludes1 note generally tells you that the excluded code should not be used together with the code above the note. The official guidance includes an exception when the two conditions are unrelated. If the relationship is unclear, query the provider rather than deciding from the code titles alone.

This makes a simple “never together” shortcut incomplete. The documented conditions and the actual exclusion must be reviewed together. Do not assume that the presence of two different descriptions proves the conditions are unrelated.

An Excludes2 note identifies a condition that is outside the meaning of the code being viewed. A patient may have both conditions, and both codes can be reported when appropriate. The note is not an instruction to add the second code automatically.

First establish that both conditions are documented and meet the reporting rules for the setting. Then check any other instructions that affect assignment or sequence.
